I have received two of these forms along with this year's property valuations (done every 4 years I think).  Hitherto these have been free but now the hacienda want 60 Euros for sending you the information - like you have a choice.  Take the two copies down to the bank and pay up or, surprise surprise, the bill increases. 

I don't suppose you writing back to them and enclosing a bill for 50 Euros for doing so would mean you only have to pay them a tenner.   :)
