I think you can find the form on the website of the government:

http://www.mir.es/SGACAVT/modelos/extranjeria/modelos_extranje/

Maybe Michael can confirm if the ex-16 form is the one you need, the form also gives a list of the needed documents. On the same website you can find lots of other forms for extranjeros.
